WITH THE PRACTICE?
To register with the practice,
please bring proof of address in our catchment area, your NHS medical card together with details of your previous address
in the UK and of your General Practitioner. (Without this information we will
be unable to register you.) If you don't have a medical card, you will be given
a registration form to fill in which will complete the registration process with
the practice.
It is important that we know about any relevant medical condition or history
you may have. Therefore, if you are on regular medication or have an ongoing
medical condition, please make an appointment to see one of the doctors as soon
as possible.

Please notify our reception staff of any change of name, address or telephone
number in order to ensure that your medical records are correct.
